聯合國兒童基金會估計，全世界有45億人沒有安全可靠的公共衛生系統，這也導致每年有48萬名五歲以下的孩童死於痢疾，主要在南亞和薩哈拉非洲。根據劍橋經濟學院與日本馬桶製造商儷住(Lixil) 做的研究報告，光是2015年，人類因為沒有公共衛生系統而付出的成本高達2230億美元。
